LifeStory Connection™ is a new, innovative software application to support person-centered care. This application conveys a persons life story and care plan strategies to care partners to personalize the healthcare experience for the most dedicated providers. The app is view-only and displays real-time information on a phone or tablet in a simple, easy format.
The LifeStory Connection™ application builds trust, improves engagement, and helps to yield better outcomes. The use of this application has helped care partners improve their knowledge of resident’ life stories and thus, improve the overall resident experience through their healthcare journey, especially those with dementia.
Resident life story information displayed on this application include:
Family
Life history (occupation, etc.)
Dining preferences
Favorite activities (music, pets, sporting events, etc.)
Important preferences (routines, things that upset them, etc.)
My Pref (their preferences related to dressing, etc.)
This application includes a TOP 15 display for each person as well as an "ALL INFO” feature to display their entire life story. The user can scroll through all of these navigation options to familiarize themselves with the person.
