LifeStory Connection app for iPhone and iPad

The LifeStory Connection™ application is a new, innovative means to convey life stories and care plan strategies to care partners supporting persons with Alzheimer’s and other dementia. The app is view-only and displays real-time information on a tablet in a simple, easy format to help care partners facilitate a person-centered approach to dementia care.

The LifeStory Connection™ application builds trust, improves engagement, and yields better outcomes. The use of this application has helped care partners improve their knowledge of resident’ life stories and thus, improve the overall resident experience through their journey with dementia.

Resident life story information displayed on this application includes:


- Family information
- Life history (occupation, religion, veteran status, etc.)
- Dining preferences
- Favorite activities (music, pets, sporting events, etc.)
- Important preferences (routines, things that upset me, etc.)
- My Pref (their life story related to personal care)

This application also includes a TOP 15 display for each resident as well as an "ALL INFO” feature. The user can scroll through all of these navigation options to familiarize themselves with the resident.

HIPAA (Health Insurance Portability and Accountability Act) regulations protect the privacy of health information. This application displays a persons life story and does not include any health-related information.
